Application Requirements
Applicants must submit the following documents to be considered for the Student Assistantship Program:
- Application letter addressed to the Office of Student Affairs (OSA)
- Updated resume or Student Assistant Information Sheet (OSA Form).
- Accomplished Parent Consent and Undertaking Form for Student Assistantship.
- Student Assistantship Contract.
- A copy of the registration form with the class schedule.
- Copy of previous semester’s grade.
Please hand in your application to the Office of Student Affairs, addressed to:
MRS. DAISY S. JUDAVAR
Dean of Student Affairs
*Application letters with incomplete documentary requirements will not be received.
